CareGap Home Health welcomes referrals from physicians, hospital discharge planners, case managers, social workers, community partners — and families referring a loved one.
Who You Can Refer
- Children under 21 who are medically fragile and may qualify for GAPP (Georgia Medicaid — no cost to eligible families)
- Adults and seniors who need skilled nursing, personal care, companion care, or light housekeeping at home
- Patients being discharged from hospital or rehab who need support transitioning home safely
How It Works
Send us the referral below or call (706) 870-5182. We respond within one business day, complete a free assessment, and keep you informed of the outcome. Our pharmacist-led care model means every referral gets a medication-safety review as part of the care plan.
