As a Georgia GAPP-enrolled Medicaid provider, we proudly provide skilled nursing and personal support services for medically fragile children with complex healthcare needs. We also serve adults and seniors who require compassionate in-home care due to aging, disability, chronic illness, or recovery following hospitalization.

CareGAP was founded by Krystal Ikemba-Madu, PharmD, a licensed pharmacist with more than 10 years of patient care experience. After witnessing the challenges families faced with medication management, hospital-to-home transitions, and finding dependable caregivers, she envisioned a different kind of home care agency, one that combines compassionate caregiving with clinical expertise.

Today, that vision continues through our pharmacist-informed approach to care. Every care plan is developed with an emphasis on safety, continuity, and collaboration, helping ensure that caregivers, families, and healthcare providers work together to support the best possible outcomes.
